The Cabrini Partnership Permanent Supportive Housing Program follows a consumer preference supportive housing model; therefore, affordable one-bedroom/efficiency apartments are available to people who are long-term homeless, living on the streets or in emergency shelters. Many of these individuals have severe and persistent mental health and chemical dependency issues. In addition to secure housing in scattered sites throughout the Twin Cities, such as the Dundry Apartments and the Van Cleve Development, the program provides ongoing case management. During FY 2009, this program served 117 individuals.
Activities
Each resident works in collaboration with a case manager to establish goals and objectives in each area of his/her life including basic needs, education, employment, family and health. The goal plan is monitored and revised when goals are met or if a resident wishes to work in other areas. Cabrini Partnership works with many different agencies to ensure external resources are available for residents to achieve their goals.
Education and Employment-related Services
Many Cabrini Partnership residents have poor work histories, gaps in employment due to illness or homelessness and criminal histories. They often lack adequate education and job training that would qualify them for more than minimum wage. Cabrini Partnership works closely with each individual to identify career and vocational interests and overcome work history problems by developing a new track record. Cabrini Partnership and the resident develop a strategy for gaining the education and/or employment that will provide a living wage.
Independent Living Skills Development
During the planning phase, case managers assists participants in becoming accustomed to their neighborhood and its bus lines, grocery stores, laundry facilities, health facilities and support groups. Together, case managers and participants develop a working budget and monthly rent payment plan to help re-establish financial responsibility.
Family Reunification
Restoring relationships with family is a critical ambition for many of our residents. Many of those we serve are parents who have lost temporary custody of their children or whose children are living with a relative.
Cabrini Partnership addresses parents’ needs, and we promote healthy family relationships. Residents are encouraged to attend parenting classes and support groups. Understanding age appropriate behavior and increasing communications skills are among the goals residents pursue.
Mental Health Support
Case managers help residents identify appropriate medical/mental health providers. Through frequent discussions, case managers assist residents in recognizing warning signs and triggers, and they identify steps to prevent crisis. The goal is to help clients see the value in adherence and commitment to mental health regimens.
Sobriety
Cabrini Partnership utilizes clinically based best practices to support mental/chemical health recovery. Accessible support and regular staff interaction provide practical feedback for residents to recognize and avoid vulnerable or risky situations and to prevent relapses. The staff is versed in abstinent approaches and harm reduction interventions to facilitate successful community living.
